Hi - where can we download some of these recommended forms? My son (with several food allergies) is starting daycare this month and I would love to have a form to fill out instead of creating my own health care plan for him.

Hello @Elizabeth Marn , you can find free PDF versions of some of these forms on the AAFA store , like the Anaphylaxis Action Plan . Other forms, such as a medicine authorization form or special dietary meals accommodation form, can typically be obtained from your child's school.
